Under this model, if a patient receives seamless, coordinated, and effective care—from hospital admission through rehabilitation and recovery at home—providers are rewarded with higher Medicare reimbursements. Conversely, fragmented or inefficient care that leads to complications or readmissions results in reduced payments.
