Paradise Valley Hospital is excited to offer a secure and convenient way for patients to access and manage their health information online. With a personalized account, patients can now view their information online anytime.
Patients get secure and convenient access to:
- medical record summary
- clinical results
- medication history
After activation of an account, patients can access Paradise Valley Hospital’s patient portal by visiting the hospital or clicking here. Once patients access the portal’s home page, patients enter their username and password.
Typically, the test results will be available within 36 hours of discharge from the hospital.
After hospital discharge, a Continuity of Care Document (CCDA) will be available in the patient’s document folder. This document includes details of the care available at the time of discharge, including:
- allergies
- discharge medications
- procedures history
- plan of care
- hospital course
- diagnosis
